Meet Our Caregiver of the Month

NovCaregiver2015Sherry Duran is the winner of the Hospice Caregiver Award, presented by Horan & McConaty. Sherry was nominated by a patient’s family member, Pedro Mondragon.

We want to thank Sherry Duran for always being there for us.  This woman is amazing.  All of her duties & responsibilities that she does endure.  She still has empathy and cares so much about our family.  She is smart and very aware of all concerns we have about our dad.  Her advice is so comforting and her help with everything is done ASAP. We all love her and her team has been so remarkable. She is a wonderful leader and a great person. We can count on her and she will problem solve and be there for us. We are grateful for having her for our dad.

We love her for everything and we are glad to have her.

Dame Cicely Saunders stated “We do all we can not only to help you die peacefully, but also to live until you die.” Kevin Samelson reminds us daily by not only being so caring, compassionate and competent but also by going that extra mile for each individual, all his patients experience Saunders vision. It is a privilege to have him on our team. I have booked him for my own departure.

Julie Juarez of Halcyon Hospice is the winner of the Hospice Caregiver Award, presented by Horan & McConaty.  Julie was nominated by Jenn Davis.

I am the Team Lead for Julie Juarez and was recently visiting a patient’s home. The patient we were caring for has had several caregivers in and out of the home, as many were not “good fits” and it was difficult to match personalities and approaches for this family. During that visit, the patient’s mother looked at me in the eye and said, “You treat her right, she is a jewel”. I acknowledged that comment and then she adamantly stated, again, “No, I’m telling you, she is the best employee you could have”. I knew this about Julie, but this statement coming from a family member spoke volumes. I have been observing the types of cases that Julie is assigned to and the intensity of the clients she serves. She is consistently able to care for people who others have difficulty caring for. Through her selfless approach, she is able and willing to serve a population that needs her care the most. She defines professionalism while creating a warm, accepting environment for all of her patients and colleagues. Julie is the true definition of a Servant Leader, one who humbly walks with others, regardless of who they are, offering her presence and genuine care. I do not make nominations lightly, but not only have I recognized Julie’s care, in only three days four of Julie’s colleagues have complimented her willingness to serve. I feel, at my core, that Julie should be publicly recognized for the care she provides to not only our patients, but her colleagues and community. I am honored and humbled to work with her, as I know her colleagues are, too. As a nurse in one of the nursing homes Julie goes to says, “She’s a shining star”.

August Caregiver of the Month

Debi Cannon is the winner of the August Hospice Caregiver Award, presented by Horan & McConaty.

Debi was nominated by Lynn Fraser.

Debi Cannon cared for my mother every day for well over a year and became my (paralyzed) mom’s lifeline. Debi would assist my mother in every task including hygiene tasks. Mother was always treated with dignity and respect. Debi bathed my mother one hour before she passed. I believe to this day, my mom was waiting to see Debi one last time before she left us. Since my mom could not use her limbs, Debi would assist Mom in ways that I could never repay. My mother cherished her, and confided in her. Ms. Cannon was professional, reliable, hard working, and trustworthy. Although the nursing home where my mother resided was full of drama from the hired staff, Ms. Cannon remained mature and professional. What’s more impressive – she gained the respect of the nursing home staff and Physicians without joining in the drama. If you have an award for an outstanding caregiver then Debi Cannon absolutely deserves it. My mother died in January, (your Funeral Home handled Mother’s body and cremation). My mother was my best friend and losing her is devastating. But caregivers like Debi Cannon gave Mom a quality of life she would never have had otherwise in her final years. Please consider this very caring and dedicated hospice caregiver. She is priceless!

July Caregiver of the Month

HMC-0378-FB-Caregiver-JulyNancy Haas of Gentiva Hospice of Denver is the winner of the July Hospice Caregiver Award, presented by Horan & McConaty.  Nancy was nominated by Nathan Gustafson, M.Div., SCC & Volunteer Services.

Nancy Haas has served with Gentiva hospice throughout the last year, first as an intern and over the last six months as a specialized Bereavement volunteer. When our Bereavement Coordinator went out on medical leave, Nancy stepped in and has dedicated 10 hours or more a week contacting our bereaved families, meeting with community members seeking support and ensuring that families receive the highest level of care. She serves with a smile and with laughter and has brightened our staff with her positive attitude. She doesn’t seek attention for her service, but has been a life saver for us in this time.

June Caregiver of the Month

Theresa Conde is the winner of the June 2015 Hospice Caregiver Award, presented by Horan & McConaty. Theresa was nominated by Desiree Lurie.

Theresa Conde is a wonderful CNA (and team member.) She has a very calm and gentle manner which inspires confidence in even the most particular patient… of which she has had a few! She is able to deal with folks who have dementia and encourage them to take a shower or get cleaned up with her gentle manner. Theresa has experience with comfort touch and is a certified massage therapist. As such she brings a lot of healing and comfort to her patients. I know families are always very happy to have Theresa care for their loved ones… old or young.

May Hospice Caregiver Award

MayCaregiver2015Carol Robertson is the winner of the May Hospice Caregiver Award, presented by Horan & McConaty.  Carol was nominated by Dina Schall.

I had to write and tell you how exceptionally kind and caring my mom’s CNA, Carol Robertson, is.  My mother was diagnosed almost 10 years ago with Alzheimer’s.  Her doctor put her on hospice care in January 2015.  Mom had a hard time connecting with any caregivers for years. I figured it was due to her disease.  Upon suggestion from mom’s social worker, we decided to try Carol.  Within five minutes of Carol walking in the door, we heard Mom laugh and saw her smile!!  I was amazed!  She showered her, dressed her, fed her and held her hand the entire time.  I knew then she was sent from heaven!  The second time Carol was here, I even heard my mom singing in the shower.  After all these years with the disease, Mom doesn’t remember anything.  She hasn’t known who I am for many years.  There is something special with Carol.  Mom couldn’t tell you her name but she seems happy and content when Carol is near.  Carol never fails to give Mom a hug, even when Mom is having a very bad day.  She… Carol… is the most devoted and caring person I think I have ever met.  I don’t know how much time I have left with my mother but I will be eternally grateful for the time we both have had with Carol Robertson.

Meet Our April Caregiver of the Month

AprilCaregiver2015aRosemary Sleevi is the winner of the April Hospice Caregiver Award, presented by Horan & McConaty.  Rosemary was nominated by Jackie Foglio, LSW, Halcyon Hospice.

In a nutshell, being around Rosemary Sleevi brings a smile to everybody’s face. She is kind, caring, helpful, funny, and has a wonderful demeanor/calming presence about her. Not only has she been a huge asset to our south office and our bereavement program, willing to help out wherever she can, she does an amazing job out in the field with patients, doing energy work, massage and aromatherapy. She has brought peace and relief to patients, caregivers and staff alike. She is a complete badass, and I feel honored to have her on our team!

MarchCaregiver2015Jennifer Moss, LCSW at Suncrest Hospice, is the winner of the March Hospice Caregiver Award, presented by Horan & McConaty.  Jennifer was nominated by Judy Jones, LCSW Director of Social Services at Suncrest Hospice.

Jennifer Moss represents hospice social work to the fullest. Her many years of experience are evident in her knowledge of resources, ability to problem solve and provide compassionate presence with our patients and families. She moves easily between the “head and the heart” – both so important in making end of life decisions. Jennifer’s passion is teaching about end of life issues, whether that is students in the classroom, new employees or families – she brings a wealth of knowledge and commitment all that she does. It has been one of my greatest honors to work with someone with Jennifer’s level of professionalism and all around niceness. I nominate her for Caregiver of the Month wholeheartedly.

In January we gathered all our 2014 monthly Hospice Caregiver Award winners, along with their friends and family members to celebrate twelve amazing caregivers in our community.  We shared dinner and the beautiful nominations each of them received last year. Joan Hummel from Porter Hospice & St. Anthony Hospice was named Hospice Caregiver of the Year for 2014. Congratulations, Joan, and thank you for all you do!
