Stephanie Bergslien is the winner of the Hospice Caregiver Award presented by Horan & McConaty. Stephanie was nominated by Rosemary Sleevi from Halcyon Hospice.
I want to nominate Stephanie Bergslien for the Hospice Caregiver Award. Stephanie is one of our Care Coordinators on the Maroon Bells Team serving the northern region of our service area.
I started working with Stephanie from a distance when I became the BVT coordinator. One of the functions of the Care Coordinators is to follow up in the facilities they are assigned to when one of Halcyon’s patients dies in that facility. A plant or flower arrangement is delivered along with some goodies for the staff who cared for the patient and the other residents who live in that community. Grief Support is offered to staff, family members and friends at this time. It is part of the Bereavement Plan of Care. The Care Coordinators send me an email to notify me when that visit is completed so I can document the visit and follow up with grief support if requested.
Most of the emails I receive state ‘condolence visit completed; no concerns’. Or something like ‘condolence visit completed; please contact John Doe and set up support for patient’s roommate’. Or ‘Administrator requests support for residents and staff’. I have enclosed a sample of Stephanie’s emails below. Stephanie not only knows the staff at her facilities, she is intimately involved with our patients, their families, the staff caring for them and the other residents who live in these facilities. She is truly an ambassador for Halcyon going above and beyond her job description.

Each monthly Caregiver Award winner will receive a gift card that can be used for whatever the recipient decides and an award.
At the end of twelve months the review committee will select the Caregiver of the Year to be revealed at a banquet honoring the twelve finalists. The Caregiver of the Year will win a trip for two within Colorado.
